Surgical closure of VSD - 5.6 kg / 7 month old baby

A 7 month old baby boy weighing 5.6 kg had frequent episodes of cold , cough and fever with poor weight gain. On investigating , the baby was found to have a ventricular septal defect , a hole in the heart. Looking at the clinical history , the parents were advised open heart surgical closure of VSD. As the family was financially not well to do, the surgery was performed under state Government scheme (MJPJAY) free of cost for the patient. The baby had a fast recovery and was discharged 5 days after surgery. On follow up after 3 months ,, baby is hale and hearty and gaining weight like normal kids.

CABG with Aortic valve replacement

82 years old lady came to OPD with severe breathlessness at rest, chest pain and syncope. She was diagnosed with severe aortic stenosis with critical LAD disease. Patient was apprehensive of surgery because of old age. She was counseled and she became ready for surgery. Her aortic valve was replaced with bio prosthetic valve and coronary artery bypass grafting was done for LAD. The patient recovered after surgery in excellent condition and was discharged on POD 8. This proves age, though an important factor, should not refrain patient from undergoing surgery if it’s necessary.

Acute aortic dissection

46 years old female presented to emergency department with severe tearing chest pain and uncontrolled hypertension. The CT scan suggested Type A aortic dissection which involves tear and separation of layers of aorta. She was taken for emergency surgery. The aortic valve was repaired and the dissected ascending aorta and part of arch was replaced with synthetic graft. The blood loss was minimal and patient had a fast recovery. She was discharged on POD 5 and is doing well now. So , its rightly said , a stitch in tie saves nine.

CABG in a patient with severe heart failure (LVEF 20%) with severe MR, severe TR

72 years old man, had heart failure symptoms due to critical coronary artery blockages leading to left ventricular EF of only 15-20% ( In normal people, it ts 60% ). Patient also had severe mitral and tricuspid valve regurgitation with kidney failure and fluid built up in lungs and abdomen. Patient was medically optimized pre operatively, Intra-aortic balloon was inserted preoperatively and Bypass grafting was done. Patient recovered from surgery gradually. His EF improved to 35% , MR and TR decreased to mild. Patient went home on POD 10 and is hail and hearty now.
